--> a. Preparticipation Physical Form
--> b. Emergency Information (get notarized)
--> c. UIL & CISD Athletic Policies
--> d. UIL Steroid Agreement
Complete forms A & B prior to your physical and bring them with you to your physical!
Keep ALL forms (A-D) & give them to YOUR child’s coach by the first day of
practice. To Prevent Loss of Forms, Do Not Send Forms to the
Athletic Training Office or Athletic Office
